This form is for production work to be performed by the Office of Instructional Resources multimedia staff. Please be as specific as possible in the details of your request. Submission of this form does not initiate the work. Someone from OIR will contact you regarding your request.

If you have an engineering request (installation of classroom technologies, repairs, etc.) please use the Engineering Project Request form.

If you have work orders that are not related to classroom technologies (ceiling light replacement, broken locks / doors, etc.) please use the UCF Physical Plant work order system. OIR cannot process work orders meant for the Physical Plant.
